EMERGENCY CARE: Health Improvement Institute LLC does not provide urgent care or emergency care. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

NOT A SUBSTITUTE FOR PRIMARY CARE: Our services are complementary to, not a replacement for, traditional primary care. You should maintain an ongoing relationship with a primary care physician for routine care, screenings, vaccinations, and acute medical issues.

1. Overview of Services

Health Improvement Institute LLC (“we,” “us,” “our,” or “the Practice”) provides physician-led preventive medicine and metabolic health optimization services on a direct-pay basis.

Our services include:

  • Extended physician consultations (telehealth and in-person)
  • Metabolic health assessment and optimization
  • Preventive care planning and risk stratification
  • Evidence-based medication management when clinically appropriate
  • Nutrition and lifestyle optimization strategies
  • Direct physician access for non-emergent questions

2. What We Do NOT Provide

Health Improvement Institute LLC does not provide:

  • Urgent or emergency medical care — For urgent issues, contact your primary care physician or visit urgent care. For emergencies, call 911.
  • Primary care services — We do not perform annual physical exams, administer vaccines, or manage acute illnesses. You must maintain a relationship with a primary care physician.
  • On-demand or 24/7 care — Our physician access is for non-emergent questions during business hours.
  • Specialist care — We focus on prevention and metabolic health. For specialized medical conditions, we will refer you to appropriate specialists.

3. Educational Information Only

Information provided on this website, in blog posts, or through educational materials is for general educational purposes only and should not be considered medical advice or a substitute for professional medical evaluation, diagnosis, or treatment.

Nothing on this website creates a doctor-patient relationship until you formally enroll as a patient and complete the intake process.

4. Insurance and Payment

Direct-Pay Practice:

  • Health Improvement Institute LLC does not bill or accept insurance
  • All services are paid directly by the patient
  • We are unable to see Medicare beneficiaries due to federal regulations
  • You may be able to use HSA/FSA funds — check with your plan administrator
  • Lab work and prescriptions are billed separately through standard medical channels and may be covered by your insurance

Payment Terms

  • Monthly membership fees are billed on a recurring basis
  • Payment is due at the time services are rendered for one-time consultations
  • Cancellation requires 30 days’ notice
  • Refunds are not provided for services already rendered

5. Patient Responsibilities

As a patient, you are responsible for:

  • Providing accurate and complete medical history
  • Following through on recommended lab work, imaging, or specialist referrals
  • Maintaining a relationship with a primary care physician for routine care
  • Informing us of any changes to your health status, medications, or care team
  • Understanding that our services are preventive and complementary, not comprehensive primary care
  • Seeking emergency care when appropriate rather than contacting our practice

6. Telehealth Services

We offer telehealth consultations via secure, HIPAA-compliant video platforms for patients located in states where Dr. Verma is licensed (New Jersey, New York, Pennsylvania, California).

Telehealth Limitations:

  • Physical examinations cannot be performed remotely
  • Technical issues may disrupt consultations
  • You must be physically located in one of our licensed states during the consultation
  • Telehealth is not appropriate for urgent or emergency situations

7. Medications and Prescriptions

When clinically appropriate, Dr. Verma may prescribe medications as part of your care plan. All prescriptions are evidence-based and tailored to your individual situation.

Important Notes:

  • We do not prescribe controlled substances or medications with high abuse potential
  • Prescriptions are sent to the pharmacy of your choice
  • You are responsible for prescription costs (insurance may cover these)
  • Regular follow-up is required for ongoing medication management

8. Cancellation and Refund Policy

Monthly Membership

  • Cancel at any time with 30 days’ written notice
  • You will be billed for the current billing cycle and one additional month
  • No refunds for partial months or unused services

Appointments

  • Cancel or reschedule at least 24 hours in advance to avoid a cancellation fee
  • Late cancellations (less than 24 hours) may result in a fee equal to 50% of the consultation cost
  • No-shows will be charged the full consultation fee

9. Privacy and Confidentiality

We are committed to protecting your privacy and complying with HIPAA regulations. Please review our Privacy Policy for detailed information about how we collect, use, and protect your personal and health information.

10. Limitation of Liability

To the fullest extent permitted by law, Health Improvement Institute LLC and Dr. Varun Verma shall not be liable for any indirect, incidental, special, or consequential damages arising from or related to the use of our services or website.

Our liability is limited to the amount paid by you for services rendered in the 12 months prior to any claim.

11. Medical Emergencies

IN CASE OF EMERGENCY:

Do not use email, contact forms, or patient portals to communicate about urgent or emergency medical issues.

For emergencies, call 911 or go to your nearest emergency room.

For urgent but non-emergency issues, contact your primary care physician or visit an urgent care center.

12. Changes to Terms

We reserve the right to modify these Terms of Service at any time. Changes will be posted on this page with an updated “Effective Date.” Your continued use of our services after changes are posted constitutes acceptance of the updated terms.

13. Governing Law

These Terms of Service are governed by the laws of the State of New Jersey, without regard to its conflict of law provisions. Any disputes arising from these terms shall be resolved in the courts of New Jersey.
